All Coaches and Managers are required to complete the following form.

2. Please mail a photo copy of your Driver's License to the address below.  This is needed to complete the background check. (VERY IMPORTANT)

To all Coaches, Managers, & Assistant Coaches:  FGSL registers and insures every adult who is scheduled to step on the field coaching our girls at practices and games. The insurance is provided through ASA and it is critical for the protection of the league and the adults who work with the kids. This year the system is getting better but it will require additional information from you.  ASA is going to do a mandatory background check in an effort to protect our players.  Therefore we have to provide ASA each individuals Date of Birth, Social Security #, and Drivers License #, along with a copy of each individuals Drivers License. The hard copy information will be sent to the Greater Sacramento Softball Association (www.sacramentoasa.org).  Once an individual has cleared the background check GSSA will destroy all that individuals documents.  FGSL must adhere to this policy therefore we have modified our coaches application to take this additional information from you and you will need to provide us with a PHOTOCOPY of your Drivers License.
